To assess Young Adult Carers (YACs) needs and identify the level of support they need from our services or other services in the county. The YAC Support Worker will either refer to our in house programme of one to one support, small targeted group work courses and social skills groups or they will refer and signpost to other appropriate support services. The YACs Support Worker with a team of part-time staff and volunteers will deliver a local service of one to one support including Colleges/University drop-ins, targeted group work, courses and social skills groups. YAC Support Workers will identify and assist previously unidentified carers in a variety of settings including colleges, the community, social services, and GP surgeries. As Family Support and Early Help Services develop, YAC Support Workers will work in partnership with them to ensure that they identify and support YACs and participate in multidisciplinary teams with them to support of YACs and their families. YAC Support Workers will be the face of Carers Bucks YAC service in a geographical area and their role will include identifying community assets, activities or funders which would be of benefit to carers.

Hours: 22.5 hours worked over three days a week – core hours 9.00 – 17.00 with some evening and weekend work

To deliver an effective support service by identifying and supporting carers within Amersham, High Wycombe, Wexham Park and Stoke Mandeville hospitals and to work with hospital staff to ensure that they take carers properly into account in patient care. The post will mainly focus in Wexham Park Hospital.

Location: Carers Bucks’ office in Aylesbury

To have overall responsibility for the Older Carers Support Service in Bucks and Milton Keynes for carers aged 75+ who are isolated and struggle to access existing services. To lead, manage and support the Older Carers Support Team in Bucks and Milton Keynes in the provision of timely information advice, guidance and emotional support. To hold a small case load of older carers who receive the service as agreed with the Adult Carers Service Manager. As primary care services develop, to ensure the Older Carers Support Service promotes carer awareness and referral in Primary Care Networks, GP Practices and primary care based multi-disciplinary teams.
