Bucks Charitable Alliance (BCA) is a consortium of local charities, with an excellent track record of delivering high quality services in the communities they serve. This new Social Prescribing service will be delivered by Community Link Workers working from GP surgeries in Primary Care Network (PCN) areas.

Each Community Link Worker will be directly employed by one of the three BCA organisations: Carers Bucks, Connection Support or Buckinghamshire Mind.

Social prescribing empowers people to take control of their health and wellbeing through GP referral to non-medical Community Link Workers (CLW’s) based in GP surgeries who give time, focus on ‘what matters to me’ and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support. Community Link Workers work collaboratively with all local partners.

Key Responsibilities:

  • Take referrals from GP practices within a primary care network in year 1 and expand the service to a wide range of other referrers from year 2.
  • Provide personalised, holistic support to individuals, their families and carers to take control of their wellbeing, live independently and improve their health outcomes.
  • Develop trusting relationships by giving people time and attention to their priorities and ‘what matters to me’.
  • Co-produce a personalised support plan to improve health and wellbeing, introducing or reconnecting people to community groups and statutory services.
  • Managing and prioritising your own caseload, in accordance with the needs, priorities and any urgent support required by individuals on the caseload.
  • To refer people back to other health professionals/ agencies, when what the person needs is beyond the scope of the Community Link Worker role.

Use of a vehicle is essential.

Are you a people person with empathy and the ability to build trusting relationships with older people?  If so this is a chance to be part of a new team and a new service supporting socially isolated older carers (75+).  Reporting to the Older Carers Support Team Leader you will have responsibility delivering this innovative new service in the south of Buckinghamshire as part of a team which covers Buckinghamshire and Milton Keynes.  The service involves a comprehensive assessment of the carers needs in their own homes using motivational interviewing techniques.  You will jointly develop a support plan to improve the carers health and wellbeing and reconnect them with the community.   Training will be provided but you will already have at least two years’ experience of assessment, advice and support planning.  You will be a people person with empathy and the ability to build trusting relationships with older people and local health and social care professionals.

Salary: £26,000 – £28,000 p.a.

An exciting opportunity to lead Carers Bucks young carers and young adult carers (YAC) support team.  Reporting to the Young Carers Service Manager you will manage this award winning service which supports nearly a thousand young carers and YACs in Buckinghamshire.  The service includes one to one work, small group work, work with young carers in schools, young carers clubs and a range of trips and residentials.  You will already have experience of working with children and young people and ideally have some management experience.  You will have energy, enthusiasm and empathy with an ability to build trusting relationships with children and young people.  You will also have the ability to work closely with and local health and social care professionals including partner agencies such as Early Help and the Family Support Services.

Salary: £24,000 – £26,000 p.a.

This is a chance to lead and develop a new team and a new service providing support for socially isolated older carers (75+) in Buckinghamshire and Milton Keynes.  Reporting to Carers Bucks Adult Carers Service Manager you will have responsibility for developing this innovative new service and managing two advice and support workers, one based in Buckinghamshire the other in Milton Keynes.   You will also hold a caseload.  The service involves a comprehensive assessment of the carers needs in their own homes using motivational interviewing techniques.  You will jointly develop a support plan to improve the carers health and wellbeing and reconnect them with the community.   Training will be provided but you will ideally have some management experience and will have at least two years’ experience of assessment, advice and support planning.  You will be a people person with empathy and the ability to build trusting relationships with older people and local health and social care professionals.
