Criteria and Make A Referral
Referral criteria
The young person is all of the following:
- between the ages of 8-16 years old
- lives in Milton Keynes
- is aware of the referral and parent/guardian consent has been given
- cares for a family member
The young person cares for someone with at least one of the following:
- a physical or learning disability
- a long-term illness
- a terminal illness
- a mental illness
- alcohol or substance misuse
Their caring role has a direct impact upon or affects at least one of the following:
- social inclusion
- educational opportunities and achievement
- their emotional wellbeing
- their physical wellbeing
- family relationships
How to make a referral
You can refer a young carer to our services by filling in our online referral form. Once we have received the referral, we will make contact with the family.
